A serious accident was defined as an accident required to be reported immediately to Commission Headquarters, and included any of the following: (a) fatalities; (b) government property damage of $5,000 or more; (c) an external radiation exposure greater than 15 rems received over a short period of time; and (d) other injury or industrial illness, no matter how slight, of five or more persons in one accident, and other defined accidents.
