Between 2010 and 2015, twelve incidents were reported of unauthorized disclosures of Top Secret information. Of these, two incidents involved the unauthorized disclosure of Top Secret Restricted Data, both of the TSRD incidents being reported at DOE Headquarters. Accordingly, the Secretary asked the Office of Security, in the Office of the Associate Under Secretary for Environment, Health, Safety, and Security to lead an enterprise-wide effort to assess the factors that contribute to the risk and identify options that, if adopted, could reduce the risk of unauthorized disclosures. This, this Report for The Secretary ("Report") delineates options for reducing access to Top Secret information in the Department and presents an analysis of the feasibility of implementing each option including cost drivers. The options were identified and analyzed by a working group of personnel drawn from program offices throughout the DOE enterprise. Four categories of options were identified; Replacing the Q clearance with Top Secret and retaining the Sigma system; Revising the Q Clearance process to reduce access to Top Secret information, Creating a 'ready reserve' of personnel who would be pre-qualified for a Q clearance, and reducing the number of Q clearances through a variety of administrative means.
